She is ultimately responsible, but it is also quite likely that she delegated responsibility to the managers beneath her. As she mostly took the fall for what happened, that means there are still others left that didn't take their jobs seriously enough.
That said, I don't envy them the job of having to protect someone who is keen to have large, open air events. Every one of them has, and will continue to be, a potential disaster.
